Ready to enroll your employees in health insurance?
So what does it take to offer health insurance to your employees?
All we need from you is a group census form to get started.
Costs are based on ages and zip codes of each employee that will be enrolling, so it is important we have these details to provide you with accurate costs. If you are also offering coverage for dependents, we will need each date of birth per dependent, too.
Once this is complete, please send it via email to firstname.lastname@example.org or fax it to 562.869.3553, whichever works best for you. You will receive a follow up from one of our agents within 24 hours to discuss the kinds of plans you are looking for and more information regarding the enrollment process.
Eligibility for a small business to enroll in group health insurance includes:
– At least 1 employee aside from the owner must be eligible which means he or she is a “W-2” employee/on payroll (and that is NOT a spouse of the owner).
Note: A W-2 employee is an employee who averages working 30 hours or more per week in a month.
– The percentage of eligible employees required to enroll in the plan varies according to different insurance carriers but on average, at least 70% of eligible employees must actually enroll in the plan(s) offered by the employer.
Note: If you are unsure whether you will have enough participation from employees, please reach out and provide us with numbers of potential enrollments as we are able to discuss this information on a case-by-case basis with insurance carriers.
– Enrollments are a guaranteed issue so there is no need to worry about pre-existing conditions as all employees are able to enroll as long as they are considered eligible.
Once we become your agent, we will continue to act on your behalf for ALL health insurance needs!
Health plans are recognized under metal tiers: Platinum, Gold, Silver, & Bronze. Platinum and Gold plans are higher in monthly costs, but provide more coverage when services are needed. Bronze and Silver plans are lower in monthly costs, but require more to be paid out-of-pocket when services are needed.
Types of preventive services covered under all health plans include the following: Blood pressure screening, cholesterol screening, diet counseling, diabetes screening, HIV screening, immunization vaccines, anemia screening, contraception, and many others.
Dental & Vision
We also offer dental and vision benefits to our customers and can provide you with quotes for those plans as well. Let us know if you’re interested in dental & vision quotes along with health insurance or just dental and vision plans alone!
Side-By-Side Cost Comparisons
We have the tools to provide you with side-by-side comparisons of the different kinds of plans available to you and your employees. Depending on ages and zip codes, costs will vary for every business so it is important for us to gather these pieces of information from you in order to provide you with accurate prices.
